Resumo Realizou-se um estudo transversal para estimar a prevalência de abortamento espontâneo em 360 mulheres distribuídas em área de maior e menor exposição à poluição atmosférica proveniente do tráfego de veículos na Ilha de São Luís-MA e identificação de variáveis associadas. As participantes foram entrevistadas e os dados coletados foram submetidos à regressão logística bivariada e multivariada. A prevalência geral de aborto espontâneo foi 15,83%, correspondendo a 25,56% (n=180) na área de maior exposição e 6,11% (n=180) na área de menor exposição. Associaram-se ao aborto espontâneo o etilismo materno (OR=3,11), a presença de IST na gestação (OR=2,74), viver na área de alta exposição (OR=8,32), ter sofrido violência física ou psicológica na gestação (OR=4,25) e a ocorrência de abortamento de repetição (OR=39,11). Os resultados apontam para uma possível contribuição da poluição do ar proveniente do tráfego de veículos como fator de risco na etiologia do aborto espontâneo.

Abstract A cross-sectional study was carried out to estimate the prevalence of miscarriages in a population of 360 women distributed in areas exposer to higher and lower vehicle traffic air pollution on the Island of São Luís, MA, Brazil, and identify associated variables. Participants were interviewed and bivariate and multivariate logistic regressions were conducted. The overall miscarriage prevalence was of 15.83%, totaling 25.56% (n=180) in the higher exposure area and 6.11% (n=180) in the lower exposure area. Maternal alcohol consumption (OR=3.11), STIs during pregnancy (OR=2.74), living in a high-exposure area (OR=8.32), having suffered physical or psychological distress during pregnancy (OR=4.25) and repeated miscarriages (OR=39.11) were all associated to the miscarriage outcome. The findings reported herein thus indicate potential vehicle traffic air pollution contribution as a risk factor in the etiology of miscarriages.

Resumen Se realizó un estudio transversal para estimar la prevalencia de aborto espontáneo en 360 mujeres distribuidas en un área de mayor y menor exposición a la contaminación del aire por el tránsito vehicular en la Isla de São Luís-MA e identificación de variables asociadas. Los datos recolectados en las entrevistas se sometieron a regresión logística bivariada y multivariante. La prevalencia general de aborto espontáneo fue de 15,83%, correspondiente al 25,56% (n=180) y 6,11% (n=180) en las zonas de mayor y menor exposición. Aborto espontáneo se asoció con consumo materno de alcohol (OR=3,11), la presencia de ITS durante el embarazo (OR = 2,74), vivir en el área de alta exposición (OR=8,32), haber sufrido violencia física o psicológica (OR=4,25) y la ocurrencia de abortos repetidos (OR=39.11). Los resultados apuntan a una posible contribución de la contaminación atmosférica por el tráfico de vehículos como factor de riesgo en la etiología del aborto espontáneo.

Abstract Background The relationship between Multidrug Resistant-Gram Negative Bacteria (MDR-GNB) infection and colonization in critically ill COVID-19 patients has been observed, however, it is still poorly understood. This study evaluated the risk factors for acquiring MDR-GNB in patients with severe COVID-19 in Intensive Care Units (ICU). Methods This is a nested case-control study in a cohort of 400 adult patients (≥ 18 years old) with COVID-19, hospitalized in the ICU of 4 hospitals in the city of Curitiba, Brazil. Cases were critical COVID-19 patients with one or more MDR GNB from any surveillance and/or clinical cultures were taken during their ICU stay. Controls were patients from the same units with negative cultures for MDR-GNB. Bivariate and multivariate analyses were done. Results Sixty-seven cases and 143 controls were included. Independent risk factors for MDR bacteria were: male gender (OR = 2.6; 95% CI 1.28‒5.33; p = 0.008); the hospital of admission (OR = 3.24; 95% CI 1.39‒7.57; p = 0.006); mechanical ventilation (OR = 25.7; 95% CI 7.26‒91; p < 0.0001); and desaturation on admission (OR = 2.6; 95% CI 1.27‒5.74; p = 0.009). Conclusions Male gender, desaturation, mechanical ventilation, and the hospital of admission were the independent factors associated with MDR-GNB in patients in the ICU with COVID-19. The only modifiable factor was the hospital of admission, where a newly opened hospital posed a higher risk. Therefore, coordinated actions toward a better quality of care for critically ill COVID-19 patients are essential.

RESUMO OBJETIVO Caracterizar a assistência pré-natal e verificar possíveis fatores associados à sua adequação. MÉTODOS Este é um estudo transversal realizado a partir de entrevistas com os profissionais de serviços de saúde e de consultas em documentos oficiais de mulheres assistidas no pré-natal da atenção primária à saúde na cidade de João Pessoa, capital da Paraíba, na região Nordeste brasileira. O pré-natal foi avaliado por meio de um índice com critérios referentes aos aspectos de estrutura, processo e resultado, denominado IPR/Pré-Natal. Verificou-se pelo método de regressão logística multivariada se variáveis demográficas, socioeconômicas, reprodutivas e de morbidades maternas foram possíveis fatores determinantes para a adequação do pré-natal. RESULTADOS A pesquisa envolveu 130 serviços e 1.625 pacientes da atenção primária de saúde. O pré-natal se apresentou adequado em aproximadamente 23% dos casos. Foram observadas baixas prevalências de encaminhamento à maternidade, estratégias educativas e realização de exames. As análises mostraram que mulheres não adolescentes (OR = 1,390), com maior tempo de escolaridade (OR = 1,750), maior renda per capita (OR = 1,870) e primíparas (OR = 1,230) apresentaram maiores chances de ter um pré-natal adequado. CONCLUSÕES O pré-natal, ao ser avaliado por critérios mais amplos, mostrou baixo percentual de adequação. Devem ser elaboradas estratégias que garantam o encaminhamento para a maternidade onde será feito o parto e a realização de atividades de educação em saúde e dos exames para propiciar o pré-natal adequado no município estudado. Além disso, os fatores associados à adequação devem ser levados em conta por parte dos gestores e profissionais de saúde.

Prenatal care was evaluated by an index with criteria referring to aspects of structure, process and outcome, denominated IPR/Prenatal. The multivariate logistic regression method revealed that demographic, socioeconomic, reproductive and maternal morbidity variables were possible determinants for prenatal adequacy. RESULTS The survey involved 130 services and 1,625 primary health care patients. Prenatal care was adequate in approximately 23% of the cases. Low prevalence of referral to maternity, educational strategies and examinations were observed. The analysis showed that non-adolescent women (OR = 1,390), with a longer period of schooling (OR = 1.750), higher per capita income (OR = 1,870) and primiparous women (OR = 1,230) were more likely to have an adequate prenatal. CONCLUSIONS Prenatal care, when evaluated by broader criteria, showed a low percentage of adequacy. Strategies should be developed to ensure the referral to the maternity where the birth will take place and health education activities and examinations to provide adequate prenatal care in the municipality under study. In addition, factors associated with adequacy must be considered by managers and health professionals.

Abstract The etiopathogenesis of female pattern hair loss is still poorly understood. In addition to genetic and hormonal elements, environmental factors could be involved. The aryl hydrocarbon receptor is expressed in keratinocytes and can be activated by environmental pollutants leading to alterations in the cell cycle, inflammation, and apoptosis. Here we demonstrate the overexpression of nuclear aryl hydrocarbon receptors in miniaturized hair follicles in female pattern hair loss.

Resumo O objetivo deste estudo foi analisar a partir de uma revisão sistemática o impacto de estratégias educacionais desenvolvidas no pré-natal de baixo risco em resultados obstétricos. Foi feita a busca nas bases de dados PubMed, Medline, SciELO e Lilacs por ensaios clínicos randomizados com os desfechos de nascimento: peso ao nascer, prematuridade e aleitamento materno. Foram utilizados os descritores em combinação: prenatal, antenatal visits, education, health education, pregnancy outcomes, birthweight, prematurity, breastfeeding e randomized clinical trials. Após a avaliação da qualidade, incluiu-se nove estudos. As ações mostraram-se mais eficazes quando estendidas até o período pós-parto. A maior parte delas aconteceu durante as visitas domiciliares e apresentou impacto positivo na prática do aleitamento materno e peso ao nascer. A formação de grupos de gestantes contribuiu para menor prevalência de prematuridade. A amamentação mostrou-se o desfecho mais sensível às estratégias educativas. Práticas educativas durante o pré-natal contribuem para resultados obstétricos favoráveis por minimizarem dúvidas e anseios da mulher durante o processo de gestação, preparando-a para o parto e pós-parto, devendo ser incorporadas no processo de trabalho dos serviços de saúde.

Abstract This study aimed to analyze the impact of educational strategies developed in low-risk prenatal care on obstetric outcomes from a systematic literature review. This review consulted databases PubMed, Medline, SciELO and Lilacs, analyzing randomized clinical trials with the following birth outcomes: birth weight, prematurity and breastfeeding, using the following combination of keywords: pre-natal, antenatal visits, education, health education, pregnancy outcomes, birth weight, prematurity, breastfeeding and randomized clinical trial. Nine studies were included following quality evaluation. Actions prove to be more effective when extended to the postpartum period. Most of them occurred during home visits and had a positive impact on breastfeeding and birth weight. The establishment of groups of pregnant women contributed to lower prevalence of prematurity. Breastfeeding was found to be the outcome most sensitive to educational strategies. Educational practices during the prenatal period contributed to favorable obstetric outcomes as they minimized pregnant women concerns and anxiety during the pregnancy process, preparing them for childbirth and postpartum, and should be incorporated into health services’ work process.

OBJETIVO: Desenvolver e aplicar um novo instrumento para avaliar o pré-natal com base nas diretrizes do Programa de Humanização do Pré-Natal e do Nascimento do Ministério da Saúde brasileiro, inserindo elementos quanti-qualitativos distribuídos segundo a tríade avaliativa de infra-estrutura, processo de trabalho e resultados. MÉTODOS: Este estudo transversal, de natureza analítico-descritiva e abordagem quantitativa, foi desenvolvido com 238 usuárias em 44 serviços da atenção primária à saúde do Município de João Pessoa, Nordeste do Brasil, de novembro de 2010 a dezembro de 2011. Também foram entrevistados médicos e enfermeiros envolvidos no cuidado pré-natal. O instrumento elaborado pelos pesquisadores continha 23 questões relativas a infra-estrutura, processo de trabalho e resultados da assistência pré-natal. A partir das informações coletadas, o pré-natal foi classificado pelo que denominamos de Índice IPR/Pré-Natal. Para cada questão atribui-se o valor 1 para adequado, quando em conformidade com os critérios estabelecidos, e 2 para inadequado. Com base na porcentagem representada pela soma do número de questões adequadas dos três componentes em relação ao total de 23 questões, o cuidado pré-natal foi classificado em: adequado superior (100% de questões adequadas); adequado (>75%); intermediário (51 a 74%); e inadequado (<50%). As categorias de classificação do IPR/Pré-Natal foram comparadas com as dos índices de Kessner e Adequacy of Prenatal Care Utilization (APNCU). RESULTADOS: As questões referentes ao processo de trabalho foram as que contribuíram com significância estatística para que o pré-natal fosse classificado em maior proporção na categoria intermediário pelo Índice IPR/Pré-Natal. As categorias de classificação do Índice IPR/ Pré-Natal mostraram-se consistentes para detectar as variáveis prematuridade, peso insuficiente ao nascer e não realização do aleitamento materno exclusivo. CONCLUSÕES: O Índice IPR/Pré-Natal foi eficaz na incorporação de elementos quanti-qualitativos para classificação do cuidado pré-natal.

OBJECTIVE: To develop and apply a new instrument to evaluate prenatal care based on the guidelines of the Brazilian Humanization Program for Prenatal Care and Birth, including quantitative and qualitative elements distributed according to the evaluation triad of structure, process, and outcomes. METHODS: An analytic, descriptive, quantitative cross-sectional study was carried out with 238 women receiving care in 44 primary care services in the city of João Pessoa, Northeast Brazil, between November 2010 and December 2011. Physicians and nurses involved in prenatal care were also interviewed. The instrument developed by the investigators contains 23 questions relating to structure, process and outcomes of pre-natal care. Based on the information collected, pre-natal care was classified according to an IPR/Pre-Natal index (Índice IPR/Pré-Natal, where I stands for infrastructure, P for work process and R for results). A value of 1 is attributed to each question if it complies with the criteria established for quality, or 2 if it does not comply. The percent of adequate answers in relation to the 23 total questions is used to classify prenatal care as: upper adequate (100% adequate answers); adequate (>75%); intermediate (51 to 74%); and inadequate (<50%). The classification categories for the Pre-Natal/IPR index were compared to those of the Kessner and Adequacy of Prenatal Care Utilization (APNCU) indices. RESULTS: Questions relating to the work process contributed significantly to the classification of prenatal care as intermediate according to the IPR/Pre-Natal. IPR/Pre-Natal classification categories were consistent to detect prematurity, insufficient weight at birth and absence of exclusive breastfeeding. CONCLUSIONS: The IPR/Pre-Natal index effectively incorporated quantitative and qualitative elements for the assessment of pre-natal care.

OBJETIVOS: caracterizar a assistência pré-natal em Unidades básicas de Saúde envolvendo profissionais e usuárias do município de João Pessoa-PB. MÉTODOS: estudo do tipo transversal e analítico-descritivo. A fim de caracterizar o pré-natal, um instrumento específico foi elaborado com base nas diretrizes do Programa de Humanização do Pré-natal e Nascimento reunindo questões distribuídas e analisadas segundo a tríade avaliativa: estrutura, processo e resultados. RESULTADOS: observou-se boa infra-estrutura em maior parte dos serviços; quanto ao processo, identificou-se baixa adesão aos requisitos mínimos do Programa de Humanização do Pré-natal e Nascimento (39,9%), além de baixo percentual de serviços com atividades de educação em saúde (45,5%) e avaliação interna (47,7%). Para os resultados da assistência, altos percentuais de gestantes com intercorrências clínicas (60,9%), inadequada situação nutricional (56,3% sobrepeso e obesidade) e baixa prevalência de aleitamento materno exclusivo (58,05 ± 34,19 dias) foram observados. CONCLUSÕES: aspectos de processo referentes à adesão ao pré-natal, presença de serviços com atividades de educação em saúde e indicadores de avaliação interna além de questões de resultados envolvendo intercorrências clínicas, inadequada situação nutricional na gestação e baixa prevalência de aleitamento materno exclusivo são barreiras à qualidade das ações de pré-natal no âmbito da atenção primária à saúde.

OBJECTIVES: to characterize prenatal care at Basic Health Units regarding health workers and users in the municipality of João Pessoa, in the Brazilian State of Paraíba. METHODS: a cross-sectional, analytical-descriptive study was carried out. In order to characterize prenatal care, a specific tool was developed based on the guidelines of the Humanization of Prenatal Care and Birth Program, gathering together questions under the headings, structure, process and results. RESULTS: satisfactory infrastructure was found in the case of most services. In the case of the process, the study identified a low level of adherence to the minimal requirements of the Humanization of Prenatal Care and Childbirth Program (39.92%), and a low proportion of services including health education (45.5%) and internal evaluation (47.7%). In terms of the results of care, a high percentage of pregnant women with clinical irregularities (60.9%), inadequate nutritional status (56.3% overweight or obese) and a low level of exclusive maternal breastfeeding (58.05 ± 34.19 days) were observed. CONCLUSIONS: aspects of the process relating to prenatal care, the existence of services incorporating health education and internal evaluation indicators and the issue of results involving clinical irregularities, inadequate nutritional status during pregnancy and low levels of exclusive maternal breastfeeding stand in the way of achieving high quality in primary prenatal healthcare.

Soroprevalence for Hepatitis C virus is reported as 2.12% in Northern Brazil, with about 50% of the patients exhibiting a sustained virological response (SVR). Aiming to associate polymorphisms in Killer Cell Immunoglobulin-like Receptors (KIR) with chronic hepatitis C and therapy responses we investigated 125 chronic patients and 345 controls. Additionally, 48 ancestry markers were genotyped to control for population stratification. The frequency of the KIR2DL2 and KIR2DL2+HLA-C Asp80 gene and ligand was higher in chronic infected patients than in controls (p < 0.0009, OR = 3.4; p = 0.001, OR = 3.45). In fact, KIR2DL3 is a weaker inhibitor of NK activity than KIR2DL2, which could explain the association of KIR2DL2 with chronic infection. Moreover, KIR2DS2 and KIR2DS2+HLA-C Asp80 (p < 0.0001, OR = 2.51; p = 0.0084, OR = 2.62) and KIR2DS3 (p < 0.0001; OR = 2.57) were associated with chronic infection, independently from KIR2DL2. No differences in ancestry composition were observed between control and patients, even with respect to therapy response groups. The allelic profile KIR2DL2/KIR2DS2/KIR2DS3 was associated with the chronic hepatitis C (p < 0.0001; OR = 3). Furthermore, the patients also showed a higher mean number of activating genes and a lower frequency of the homozygous AA profile, which is likely secondary to the association with non-AA and/or activating genes. In addition, the KIR2DS5 allele was associated with SVR (p = 0.0261; OR = 0.184).The ancestry analysis of samples ruled out any effects of population substructuring and did not evidence interethnic differences in therapy response, as suggested in previous studies.

O Programa de Hepatopatias do Hospital da Fundação Santa Casa de Misericórdia do Pará surgiu pela necessidade de prestar assistência a hepatopatas na região amazônica priorizando assistência qualificada, identificação das etiologias, seguimento clínico, e tratamento direcionado. Este trabalho visa descrever dados relativos à epidemiologia clínica, fatores etiológicos e análise histopatológica. Dos 1469 pacientes avaliados, através de exames clínicos, laboratoriais, endoscópicos e de imagem e/ou histopatológico, foram considerados hepatopatas crônicos 935 (63,6%). Nesta casuística, a média de idade foi 50 anos, 666 (71,2%) do sexo masculino e maior procedência de Belém. Os agentes etiológicos mais prevalentes foram alcoolismo (53,7%) e hepatites virais (39,1%). Biópsia hepática realizada em 403/935 (43,1%), demonstrou hepatite crônica (34%) e cirrose (34%) na maioria das amostras. Conclui-se, portanto, que a doença hepática crônica na região é mais prevalente no sexo masculino, sendo o alcoolismo a principal etiologia e mais da metade dos casos se encontravam em fase avançada no momento do diagnóstico.

The Liver Diseases Program of the Hospital Santa Casa de Misericordia do Pará was create because of the need to attend patients with liver diseases of the Amazônia area, taking as priority to attend with quality, diagnosis of aetiologies, clinical following and specific treatment. This study aim to describe dates related to epidemiology, aetiologics agents and histopathologic analisys. One thousand sixthy nine patients were evaluated through medical, laboratory, endoscopic, ultrasound or computadorized tomography and histopathologic examination. Nine hundred thirty five (63,6%) patients within 1469 patients were diagnose as chronic liver disease. The average age was 50 year, 666 (71,2%) were male, and the most patients lived in Belem, state of Pará. The aetiologic agents most prevail were alcoholism (53,7%) and viral hepatitis (39,1%). Hepatic biopsy were done in 403 (43,1%) within the 935 patients and the results showed chronic hepatitis (34%) and chirrosis (34%). In sumary the chronic liver disease in the amazon region is more prevail in male than female, the alcoholism is the principal aetiologie, and the most of these cases were diagnose in the severe phase.

Com o objetivo de contribuir para um melhor conhecimento do envolvimento das infecções pelos vírus das hepatites B e C, na etioepidemiologia do CHC na Amazônia Oriental, estudou-se 36 pacientes em Belém-PA. Foram avaliados marcadores sorológicos e a pesquisa do HBV-DNA e HCV-RNA pela reação em cadeia da polimerase. Observou-se etilismo em 33,3% e cirrose em 83,3%. Marcadores sorológicos das infecções pelo HBV e HCV foram encontrados respectivamente em 88,9% e 8,3%. O HBsAg foi encontrado em 58,3%; anti-HBc em 86%; anti-HBe em 85,7; HBeAg em 9,5%; anti-HBc IgM em 57,1%. O HBV-DNA foi detectado em 37,7% e em 65% dos HBsAg positivos; o HCV-RNA em 8,5% e em 100% dos anti-HCV positivos. AFP esteve alterada em 88,9% e acima de 400ng/ml em 75% dos casos. Conclui-se que a infecção pelo HBV parece ter importância na etiologia do CHC e ressalta-se a importância de implementar programas de vacinação e detecção precoce do tumor.

In order to contribute to a better understanding of the possible role of hepatits B and C in the etiopathogenis of HCC in the East Amazon, there were studied 36 patients in Belém/PA. Serological hepatitis markers were evaluated and polymerase chain reaction assays were used to detect HBV-DNA and HCV-RNA. Alcohol abuse was observed in 33.3% and cirrhosis in 83.3%. In 88.9% of the sample, one or more hepatitis B markers were positive. Also, 8.3% those patients had anti-HCV simultaneously positive. The HBsAg serological test was positive in 58.3%; anti-HBc in 86%; anti-HBe in 85.7%; anti-HBe in 9.5%; IgM anti-HBc in 57.1%. The HBV DNA was found in 37.7% and in 65% of the HBsAg positive. The HCV RNA was detected in 8.5% and in 100% of' the patients positive to anti-HCV. The AFP was above the normal value in 88.9% of patients, with levels up to 400ng/ml in 75% of them. In conclusion, hepatitis B virus infection seems to be important in the etiology of HCC and improving measures such immunization and screening in the risk population should be emphasyzed.

A infecção pelo virus da hepatite B apresenta amplo espectro de manifestações clínicas. Objetivando conhecer os genótipos do HBV mais prevalentes e determinar a ocorrência da mutação pré-core A-1896, em uma população da Amazônia oriental, correlacionando com o diagnóstico clínico, foram selecionados 51 pacientes portadores crônicos de HBsAg e HBV-DNA positivos e divididos em três grupos: grupo A (n=14, pacientes assintomáticos); grupo B (n=20, sintomáticos HBeAg positivos) e grupo C (n=17, sintomáticos HBeAg negativos), sendo usado o sequenciador automático ABI modelo 377 para identificação de genótipos e mutantes pré-core. Os resultados evidenciaram o genótipo A como o mais prevalente, 81,8%, 89,5% e 93,7%, nos grupos A, B e C, respectivamente. A mutação pré-core A-1896 foi encontrada em 11,5% (3/26), sendo todos assintomáticos. Concluiu-se que na população estudada o genótipo A foi o mais prevalente e houve baixa ocorrência do mutante pré-core A-1896, ambos não se constituindo fatores agravantes da doença hepática.

Hepatitis B virus (HBV) infection presents itself with a variety of clinical manifestations. The present work aims to describe the prevalence of HBV genotypes and the occurrence of precore mutation A-1896 in a population group of the Eastern Amazon region of Brazil and to correlate them with the clinical presentation of chronic HBV infection. 51 HBsAg carriers (HBV-DNA positive) were selected and divided into three groups: A (14 asymptomatic subjects), B (20 HBeAg positive symptomatic patients) and C (17 HBeAg negative symptomatic patients). Using an automa ed DNA sequencer ABI model 377 by sequencing for determined of genotypes and precore mutation. The results showed that the genotype A was the most commonly found (81,1%, 89,5% and 93,7% in groups A, B and C, respectively) and precore mutation A-1896 was described in 11,5% (3/26) of group A subjects. Genotype A of HBV was the most prevalent (89,1%) and low occurrence of precore mutation A-1896, both not associate with the worst outcome of the chronic infection of HBV.
